We create a culture of respect, trust and understanding for the highly diverse population we serve in our communities.The Public Health Division’s Chronic Disease Prevention and Health Promotion (CDPHP) Unit is seeking a Prevention Coordinator to join their Health Prevention and Promotion team. This Coordinator will be responsible for streamlining and integrating efforts to reach and educate residents across the life-course about primary prevention.
The program's main focus is on engaging and supporting Black, Indigenous, People of Color, Immigrants and Refugees, LGBTQ2SIA+, and other priority communities to prevent substance and drug use and misuse, and the development of substance use disorders. By working collaboratively with a diverse range of stakeholders, the Coordinator will help to ensure that substance use prevention efforts in Multnomah County are effective, equitable, and responsive to the needs of the community.
The Coordinator will focus on identifying successful primary prevention strategies through a shared risk and protective factor framework, elevate and identify culturally specific strategies and interventions, evaluating existing policies, managing procurement, expanding prevention activities for youth, communities and families, and implementing primary prevention and harm reduction strategies. They will work closely both in person including community settings and virtually including and not limited to culturally specific providers, community organizations, youth serving organizations, faith based institutions, leaders, and advocates to identify and address the unique needs and challenges faced by these communities.
This position requires advanced knowledge, skill and ability in working with the traditions, culture, customs, values, including historical trauma of engaging with government systems and funded services of individuals and families who identify with the following cultures/communities: Black, Indigenous, and/or LGBTQ2SIA+ .
This position requires in-person and on-site work at county buildings for internal and external meetings, community events, outreach and engagement activities, and other instances where the need of the work is best met in-person and on-site. The position is a hybrid telework position and will include some routine work time at the assigned place of work.
As the Prevention Coordinator, your responsibilities include:
Partnership Development
Evaluate existing policies and processes.
Coordinate culturally specific forums and communications.
Plan programming for target populations using a variety of educational approaches.
Respond promptly to community members seeking substance use, primary prevention, health promotion information and resources.
Cultivate positive and productive relationships with new partners implementing prevention initiatives.
Collaborate to develop, train the trainers and maintain a library of training to raise awareness of substance use, its contributing factors and available health promotion and prevention efforts.
Capacity Building
Coordinate naloxone distribution and training in county and community sites.
Partner with procurement contacts for naloxone and drug disposal products distribution.
Develop and implement naloxone training sessions with existing coalitions.
Gather feedback to modify or improve processes, training content, and delivery methods.
Establish and maintain safety plans for managing opposition to distribution or training events.
Document the return on investment of chronic disease prevention and the effect
Integrate information about substance use and primary prevention into white papers, issue briefs, fact sheets and other virtual and in-person educational events
Lead efforts to raise awareness about the connection between social determinants of health and health equity in chronic disease
Work with strategic partners to promote the value of primary prevention
Procurement, Contract Management and Evaluation Support
Collaborate with staff on contract development and procurement.
Collect and report data for funding, grant, or regulatory requirements.
Coordinate with funding sources on requirements and changes.
Procure services aligned with community needs.
Monitor subcontracts for timeliness, accuracy, and reporting.

Minimum Qualifications/Transferable Skills*:
Education/training: equivalent to a bachelor's degree and
Experience: one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.
KSA: Knowledge of Primary Prevention, Naloxone distribution and training for overdose prevention; Experience with initiatives for substance use prevention and harm reduction; Background in adult learning and youth development; Proven ability in outreach and promotion of community health.
KSA: This position requires advanced knowledge, skill and ability in working with the traditions, culture, customs, values, including historical trauma of engaging with government systems and funded services of individuals and families who identify with the following cultures/communities: Black, Indigenous, and/or LGBTQ2SIA+
The ability to become a Certified Prevention Specialist within 2 years of hire ( provided through the County)

Advanced knowledge and experience working with Black, Indigenous, People of Color, Immigrants and Refugees, LGBTQ2SIA+ communities.
Proven ability to engage with diverse community members, leaders, and organizations to build trust, relationships, and facilitate collaboration.
Familiarity with health disparities affecting these communities, including social determinants of health and systemic barriers to health equity.
Ability to work in a hybrid work environment, with the ability to attend community events within diverse community based settings and participate in virtual meetings.
Ability to speak for 6 hours or more per day.
Strong understanding of cultural nuances, values, and historical context of these communities, with the ability to integrate cultural sensitivity into program planning and implementation.
Demonstrated experience working directly with these communities in Multnomah County.

Hybrid Telework: This position is designated as “hybrid telework,” meaning you will be working both remotely (from home) and in person at an onsite location.
